Ronin of Kirigakure has dormant Forgotten Etokugan.
Kekkei Genkai Name: Etokugan ("Perception Eye")
Physiological Attributes: When activated, this causes the eyes to become a sparkling jade green with tiny black rings around the center that are said to "fluctuate" in and out of existence. The tiny black rings determine the user's ability with the Doujutsu, with one ring being visible for the most basic version, and three for a master's version. The Etokugan can evolve into the "Forgotten Etokugan" which is only seen twice every six centuries. The Forgotten Etokugan causes the pupil to increases and decrease in size at a very rapid rate, and the three black rings turn into one large ring that spins around the shifting pupil at a rapid rate. Shinobi that are known to exhibit the Etokugan often come from families that are trained in weapon arts, and it has been known to appear in young men and women from all over the world. From observing the Doujutsu, it has been noticed that it is more common to appear in young men or women that have black eyes.
Special Abilities: The Etokugan is also known as the Perception Eye and in all of it's forms it has demonstrated two abilities. The first is the "sixth sense" a phenomenon that causes members who have activated the eye to have an itching like sensation in the back of the skull, this sixth sense can alert members when something is going on that is unseen by the naked eye, and while only people with three rings or the Forgotten Etokugan can usually pinpoint the source, this ability can prevent the user from being put into a situation that is not beneficial to him, sometimes this ability can be used to predict attacks, but at the lower levels the itch comes at a point where it is much to late to react, for users with the higher levels of the Doujutsu, the itch can warn them off attacks a few seconds before they happen, but it would still take great physical strength and speed to dodge the attack.
The second ability of the Etokugan allows the user to see physical damage that is not seen by the naked eye, for instance internal bleeding or someone cut by a Chakra Scalpel. The injuries can be seen in a brilliant red hue. This ability can also see damage that is external, but it allows the user to see how deep the damage goes in the body. For someone untrained in anatomy or Medical Ninjutsu, this wouldn't really help you be able to tell if you have any damage to major organs or other important body parts, but a good indicator is "the deeper the color is, the worse it is." The ability can even show diseases of the body such as cancers of other harmful diseases, but this takes years of practice and has never been seen in someone who did not have at least two black rings.
Stages: The stages of the Etokugan have already been explained but for reference, the Doujutsu's strength is determined by the number of tiny black rings that surround the pupil, with one ring being the weakest and three being the strongest. It is possible to evolve the Etokugan into the Forgotten Etokugan which makes the pupil fluctuate in size and creates one large black ring surrounding the pupil. However, this trait only appears on average in one person every three centuries. Although anomalies have occurred.
Method of Obtainment: The Etokugan can only appear after the user has gone through a very traumatic battle that results in either near death or no chance of victory, you have to be pushed into the deepest corner with no way out and then suddenly it's as if a light appears at the end of the tunnel, and everything suddenly makes sense again. This is the feeling described by people who have activated the eye.
Limitations/Drawbacks: Unfortunately, those who overuse the Etokugan can develop intense brain tumors that result in death. This usually happens after years and years of using the Doujutsu. Users of the Doujutsu also often complain of stress on the eyes and pain from continued use of the Doujutsu in a short period of time, this pain can get extremely incapacitating, but will go away after a few days.
